deductions will be taken from their weekly paychecks. The

5. HEALTH AND LIFE INSURANCE-
Employees become eligible for Health and Life coverage after their
60th day of employment, provided they are insurable. New employees
who request coverage and qualify will be required to pay for
coverage during the first 12 months of employment. After 12
months of employment, the company will provide individual health and
life insurance coverage for eligible employees who want it at no
cost to the employee. Coverage for dependents is available, at
the employee's expense, if desired. At 5 years of employment
the company will continue to provide coverage for the employee and
in addition, will provide one-half the cost for dependents.
After 10 years of employment, the company will provide coverage for
the employee and dependents.
